Alba is a turbulent world, home to four tribes, namely the Tardia, Primus, Sycax and Forlorners. For twenty years Alba has been ruled by The Thirteen; a group of powerful and magical Tardia who rule for their own ends and have oppressed all four tribes. The tribes all have their own problems and harbour a mutual distrust of each other. The prophecies state that the Medi, a hybrid of a human and a Tardia, will unite and bring peace to Alba, but only the Forlorners know how it will do this and they are inaccessible. To make matters worse, The Thirteen are determined to seek out and destroy the Medi, before it seeks out and destroys them.
The Medi begins with Sarah, a school girl on earth, who lives with her difficult Gran. On her seventeenth birthday Sarah undergoes a physical change which she is unable to understand. When she goes to talk to her friends, Jack and Emma, about it she finds that they have left suddenly. Sarah contemplates running away, when a horrific event leads to her being taken to Alba by a Tardia Resistance cell, led by the formidable Atlo.
During her stay with the Tardia resistance, she learns more about the strange world in which she finds herself. She finds out that Jack and Emma were her protectors on earth and are members of the Primus tribe. Atlo discovers that the Tardia resistance has been betrayed and sends Sarah on a perilous journey to Primus. On arriving in Primus, Sarah is reunited with Jack and meets Sam (the Medi expert). The story then turns to how she adjusts to life in Primus, her relationships with its inhabitants and her training regime under the sadistic and deluded Raph.
The Thirteen know that Sarah is in Alba and that she is living in Primus. Firstly they manipulate Raph to try and kill her. Sam comes to Sarah's rescue and the relationship between Sam and Sarah blossoms, much to the annoyance of Jack. Sarah finds herself with many demands on her. Not only must she go to Sycax to find the Forlorners, she has to deal with the rival attentions of Jack and Sam and stave off The Thirteen’s attempts to kill her.
