Our Plan - Planning the Journey


OUR PLAN helps family and Carers to support someone living with dementia. It does this by helping them consider what may happen, and to make a clear record of their decisions. In six step it guides you to prepare and plan for the journey and provides you with some forms which will help you record your wishes for how you would like to be cared for in the future.
