Introduction There is a continuing trend in the health care industry towards a consumer driven market. Government health insurance plans, private health insurance carriers, employers and unions all have a vested interest in controlling spiraling costs and understanding treatment efficacy.1,2 To help them in their decisions, third party payers such as Workers' Compensation boards and automobile insurance corporations presently require the attending clinician to provide documented evidence of a patient's response to care over time. There was a time when the expressed opinion of a doctor was all the evidence that was required.
