In 2002, the Commonwealth of Pennsylvania established the Patient Safety Authority (PSA), an independent state agency created under Act 13, the Medical Care Availability and Reduction of Error ("MCARE") Act. The purpose of the PSA is to implement measures to reduce and eliminate medical errors and promote patient safety in hospitals throughout Pennsylvania. One of these measures includes the implementation of PA-PSRS, the mandatory statewide Pennsylvania Patient Safety Reporting System.
