INTRODUCTION Incomplete medication histories may lead to medication errors and adverse drug events, but also to a lack of information concerning the present complaint or past medical history. Withdrawal syndrome may occur if some drugs are omitted during hospitalisation. Moreover, some admissions are related to drug aetiologies (1). Allergy and drug intolerance should be documented including the importance and type of previous allergic reactions, circumstances of apparition and evolution, in order to avoid new reactions or cross allergies during hospitalisation (2, 3). Finally, over the counter (OTC) drugs and phytotherapy may cause side effects and interactions with other drugs and therefore should be included in the drug history report (4).
