Recently, AAOS Now editorial board member Howard R. Epps, MD, conducted a roundtable discussion with three leaders in the orthopaedic patient safety movement: James H. Herndon, MD; Norman A. Johanson, MD; and Claude Martin Jr., MD. Their wide-ranging discussion addressed the ongoing issue of wrong-site surgery, the impact on patient safety of changes in resident work weeks, and a recent shift in the approach to reducing errors. Dr. Epps: It has been more than ID years since the Institute of Medicine (IOM) published the eye-opening report "To Err is Human."
