Missing topic index
I love this book and never travel without it. I am so happy that it is now available as an iBook (one less printed book to carry around). I miss the topic index that appears in the back of the printed copies of this book. Perhaps it will be added if enough people realize that it is not included in the eBook?

Where is the topic index? It is a very important part of the book. It would get a 5 star rating if the index was added!
