In the United Kingdom, 150 000 people survive an acute myocardial infarction every year (Dalal & Evans, 2003). Coronary heart disease also kills more Australians than any other single disease. In 2006 there were 22,983 deaths, totalling 17% of all recorded deaths (Australian Institute of Health and Welfare, 2009).
