CASE HISTORY A previously healthy 71-year-old female was admitted to hospital following a presentation of acute congestive cardiac failure after a short history of worsening dyspnoea, orthopnoea and leg oedema. There was no chest pain or suggestion of infection. Her echocardiogram showed dilated ventricles with severely impaired systolic function with an ejection fraction below 20%. She was treated under a working diagnosis of ischaemic cardiomyopathy (1) and discharged home after one week with a view to outpatient angiography within six weeks.
